In shodhankarma Virechan is one of the classical therapies mentioned in Ayurvedic text and important procedure in Panchakarma. In the present pilot study is focused on Clinical Evaluation of Trivrutta Avaleha Virechan in Sthoulya. It is found that Trivrutta Avaleha Virechan is significantly effective in Sthoulya w.s.r. to Obesity.

A novel,simple, rapid, isocratic reverse phase high-performance liquid chromatographic method was developed for simultaneous estimation of andrographolide and berberine. The analysis was carried on Prontosil C18 (250 ×4.6 mm,5μ)column, with mobile phase consisting of acetonitrile and potassium dihydrogen phosphate buffer (pH 2.2 adjusted with ortho phosphoric acid) in the ratio of 30:70. The flow rate was set at 1.0ml/minand effluents were monitored at 235nm.The retention time of andrographolide and berberine werefound to be 10.08±0.2 and 12.47±0.2 minrespectively.Linear responses were obtained in concentration ranges of 5-50µg/ml, with R2 of 0.9989 and 0.9993 for andrographolide and berberine respectively. As per ICH guidelines, this method is accurate, simple, rapid and sensitive, hence can also be used in the evaluation of other formulations containing andrographolide and berberine.

Amavata is more similar to Rheumatoid arthritis on the basis of its clinical manifestation and pathogenesis.AcharyaMadhavakar mentioned first Amavataas a special disease entity in his famous treatise MadhavaNidanam. Ama (biotoxin) and Vata (biophysical force or kinetics) are plaing an important role in the samprapti (pathogenesis) of Amavata(Rheumatoid arthritis). The term Nidana means cause or etiology of the disease.Amavata (Rheumatoid arthritis) is a multifactorial disease. In Amavata, mainly sandhishula (joint pain), sandhisotha (joint swelling) and sandhigraha (stiffness of joint) lead to the restricted movements of the joints and patients become crippled gradually due to vitiation of Vata with Ama. In MadhavaNidanametiology or Nidana of Amavata(Rheumatoid arthritis) is vividly mentioned.Viruddhahara (Unwholesome Diet), Viruddhacheshta (Erroneous Habits), Mandagni (Diminished Agni), Nishchalata (Sedentary Life), Exertion immediately after taking SnigdhaAhara are specific etiological factors or Nidanas for Ama formation,Vatadosha vitiation and ultimately produce Amavata (Rheumatoid arthritis) manifestation.

In modern days, life style of the people has became fast. They have busy schedules, different types of physical and emotional stress, fasting and eating fast foods etc. Because of these reasons, many people are suffering from ardhavbhedaka. Severe pain is main symptom in ardhavbhedaka. In modern science, Migraine disease which is similar to Ardhavabhedaka roga, which affects and causes pain to the half of the head. Migraine headache is a neurological disease. A Positive family history is obtained in 70% of individuals affected by migraine. It is a wide spread in the population with varying severity. The aim of the study is to give consolation to the Ardhavabhedaka patients and helps to avoid further complication by giving nasya. Nasya is one of the Panchkarma, which is important in Urdhwa-jatrugata roga. Nirgundyadi ghrit is explained in treatment of Ardhavabhedaka,in Ashtang Sangrah,Uttarsthan. Nirgundi is vatashleshmghna ,Vedanasthapan, its Tikta- Katu Rasa and Ushna Veerya,it subsides kapha dosha. So, the combine action of Nirgundi swaras, Ghritmand and Saindhav subside vata and kapha dosh,which is useful in Ardhvabhedaka. 60 patients selected randomly after detailed examination diagnosed as Ardhavabhedaka, from Panchakarma OPD. History was recorded in a specially designed Performa. Proper plan of treatment was made and after taking written consent from the patient, single blind trial was conducted. Dose- 4 bindu, route- Nasal, duration- 13 days(every alternate day) Statistical Analysis done by using ‘z’ test. It has been seen that there is significant result in the lakshanas of Manyashool, Karnashool and Akshishool. Also highly significant result in the lakshanas of Bhrushool, Shankhashool and Ardhalalatshool. In this study, it has observed that Nirgundyadi Ghrit Nasya is more significant than Goghrit in Ardhavabhedaka.
